Those children eligible for an exam, prophylaxis (cleaning), fluoride varnish, and sealants (if they need them) are any child who provides a positive, fully completed permission slip. Miles of Smiles, Ltd. does encourage families to establish a dental home.

Those children in grades K, 2, and 6 who provide a positive permission slip but NOT fully completed will still receive an exam to stay compliant with the State of Illinois mandate.

We offer our program to children ranging from Preschool thru High School.
